EVALUATION DE LA DOULEUR POST DENTAIRE

Introduction: Dental extraction can be conceived without intraoperative pain thanks to the efficiency of anesthésia techniques. This is not the case with postoperative follow-up, which can often be accompanied by pain. Taking into account this postoperative pain, we were prompted to do sorne research on the best way for its prevention. The question is: is it beneficial to administer an optimal preventive dose of ibuprofen 400 mg on the level of postoperative pain? Methodology: This was a multi-centred, single-blind, randomized clinical trial involving a sample of patients who had undergone simple tooth extraction. This study was conducted at the Djoungolo Hospital and the Efoulan District Hospital of Yaoundé, from January 15, 2019 to March 15, 2019 (Three months). It focused on adults at least 18 years of age in apparent good health. It looked for preventive analgesia of ibuprofen taken preoperatively. Patients took ibuprofen 400 mg 5 minutes before extraction. The teeth extracted were the first mandibular and maxillary molars because of their dental age (first permanent teeth on the dental arch) and the decaying State of the latter. We used anaesthesia with vasoconstrictor for all patients that last an average of 1 hour. The extractions of these teeth lasted about 15 minutes per tooth. The postoperative pain was evaluated according to the simple verbal scale with modalities defined as: 0 = pain absence 1 = low pain2 = moderate pain 3 = intense pain 4 = very intense pain. These modalities were explained to the patients and they were asked to choose a number corresponding to their pain intensity. The periodicity of evaluation of the pain chosen with respect to the time of onset of action of ibuprofen (30 minutes).It appears that ibuprofen provides good postoperative analgesia if a preventive dose of 400mg is administered preoperatively. Results: The sample consisted of 99 patients including 52 women (52.5%) and 47 men (47.5%) male cases, a sex ratio of 0.9. The mean age of the patients was 29 ± 11 years with extremes of 18 and 81 years. The most represented age group was between 20 and 30 years old. The assessment of pain in post-operative showed that 88.4% of patients had no pain (EVS = 0), 11.6% of patients had low pain (EVS = 1). Evaluation of pain In 30 minutes post-operative showed that 90.4% of patients had no pain (EVS = 0), 9.6% of patients had low pain. Conclusion: Focusing on these results, ibuprofen can be useful as a preventive analgésie to anticipate acute pain after simple tooth extraction.
